I know what you mean about not feeling useful. It is different what we do here yet in some ways it's the same. When I work the ER most of what we see is fast track stuff. The hospital clinic is open 9-6 on M, T, W and 9-5 Th, F. The back of the clinic is just across the hall. You can make an appointment for the clinic the day you want to see someone or you can just walk in and be seen. Yet some folks still use the ER as a clinic thinking it will be faster. If an individual comes to the ER to be seen and the clinic is open-just down the hall and part of the same corporation and facility, it is still an EMTALA violation to send them there without checking them in and having a physician examine them. There is no charge if you are native. When they need a prescription we usually have the meds in the department in the pyxis, we pull it and give it to the doc who gives it to the patient then discharges the pt. If we don't have it then the chart goes to pharmacy who fills it and sends the chart back with the meds and the pt gets it right then. After hours or the weekend we get the pharmacy key out of the pyxis and the MD goes to the pharmacy and gets what they need. So their every need is provided for.
There is a need here though for our skills. When they are really needed they are appreciated. I guess in most cases they are appreciated. Hell, I don't know what I am trying to say. I guess it could be that I just took care of a drunk native woman a little while ago who knew the name some of my patients there knew me by, mother f----r, and screamed obscenities about white mf's not caring about the Eskimo's. That is not the general consensus although as in any culture there are prejudices. I truly believe we are welcomed by most and like there, when you are needed most is when we see the appreciation and get the thanks from the patients and families.
Well I don't know if I said what I set out to say but I've said enough for now. Again it is good to hear from the folks down in the tropics and to know things are same if not going well-situation normal-snafu.
